First Visit

We understand that most patients are apprehensive and we are committed to making your visit a pleasant one from the moment you walk through the door. We respect your time and privacy and seek to communicate clearly so that your visit will be conducted smoothly and efficiently. Our services are provided in an environment of optimum safety utilizing state-of-the-art surgical equipment.

Dental treatment

Park Avenue Oral & Facial Surgery, P.C. is open Monday – Friday from 8 am to 6 pm. We will schedule your appointment as promptly as possible. If you are in an emergency situation, every attempt will be made to see you that day.

Your initial appointment with oral surgeon, Ruben Cohen, DDS, will take place at his accredited oral surgery practice located on New York’s Upper East Side.  Occasionally, surgery can be performed the same day as the consultation. However, an extensive medical history or complex treatment plan will require an initial consultation and follow-up appointment(s) to provide treatment.

Please assist us by providing the following information, if possible, at the time of your initial consultation:

  1. Referral slip: Your referral slip from your dentist, if applicable.
  2. X-Rays: Please ask your dentist to forward any pertinent x-rays, or you may pick them up and bring them with you. Additional x-rays may be taken in our office, if necessary.
  3. Medications:  A list of medications and dosages you are presently taking.
  4. Medical Conditions: Information pertaining to any medical conditions (i.e. diabetes, heart disease, lung disease, high blood pressure, etc.).
  5. Insurance:  If you have medical or dental insurance, please bring your insurance card with you at your initial appointment.

All patients under the age of 18 years of age must be accompanied by a parent or legal guardian.

An initial consultation and physical examination is mandatory for all patients undergoing intravenous anesthesia. You must not eat or drink for 8 hours prior to your surgery. You will also need an adult to escort you home.

At Park Avenue Oral & Facial Surgery, P.C., you will be surrounded by an excellent  professional  staff that will address all your questions and concerns, and who will strive to  make you feel comfortable. The surgical assistants are  well-versed in  oral and maxillofacial surgery procedures and anesthesia protocols. All  staff members are very knowledgable with medical & dental insurance plans.  All staff is CPR certified, and Dr. Cohen holds certifications in ACLS (Advanced Cardiac Life Support), PALS (Pediatric Advanced Life Support), and is also a CPR instructor with the American Heart Association.

We take our surgical instrument sterilization program very  seriously. We use a Hospital Grade Autoclave on site to ensure that all instruments are properly sterilized by the highest possible standards.
